Job Overview
As a Clinical Pathway Specialist, you will partner with hospital teams to support multidisciplinary projects that improve how care is delivered across specialties.
You will work on-site with stakeholders to understand existing workflows, identify opportunities for improvement, and support the implementation of practical solutions within hospital settings.
You will:

Support the delivery of hospital-based projects focused on improving care pathways and coordination

Map current workflows, roles, and collaboration across departments

Help define project objectives and translate them into clear, actionable steps

Work closely with physicians, nurses, pharmacists, and hospital teams to drive alignment

Facilitate meetings, workshops, and working sessions with clinical teams
-
Act as a neutral partner to support collaboration and shared ownership
-
Support implementation of agreed changes into day-to-day clinical practice

Provide education and practical support where needed to enable adoption
-
Capture insights on challenges, unmet needs, and best practices
-
Share structured feedback to support continuous improvement

Collaborate with internal teams to ensure coordinated delivery

Ensure all activities are carried out in line with applicable standards and compliance requirements
Qualifications & Experience
Required:
-
Experience working within hospitals or healthcare environments
-
Strong understanding of how multidisciplinary teams operate in clinical settings
-
Excellent interpersonal and facilitation skills
-
Structured and practical approach to problem solving

Ability to translate complex situations into clear, workable actions

Fluent in Dutch and English
-
Willingness to travel across the Netherlands

Valid driving licence
Preferred:

Clinical or nursing background

Experience supporting pathway optimisation, service improvement, or hospital projects

Exposure to chronic disease management or long-term care pathways

Experience working across multiple stakeholders in a healthcare setting
